Hawaii has an employer coverage law unlike any other state and very limited inter-island specialist capacity, so both the employer question and the travel question need answering before anything else.
Most of Hawaii's specialist and hospital capacity sits around Honolulu, Pearl City, Hilo and Kailua. If you live outside those areas, the question that decides your plan is not the monthly premium — it is what happens when a referral sends you an hour or three down the road, or across a state line.
