Missouri's two metro markets sit on opposite state lines, so Kansas and Illinois providers are part of everyday care for a large share of residents.

Most of Missouri's specialist and hospital capacity sits around Kansas City, St. Louis, Springfield and Columbia. If you live outside those areas, the question that decides your plan is not the monthly premium — it is what happens when a referral sends you an hour or three down the road, or across a state line. Broad PPO networks exist precisely for that case, and a narrow local HMO usually does not cover it.
